“This grant will support Horizon Health Services’s implementation of a telehealth platform for mental health services. Horizon will implement a branded version of the American Well platform, which will help extend its clinicians’ capacity to see patients (particularly those located in rural areas) improve Horizon’s ability to document and share information through its electronic health records system.”
